If you are self-referring, you will be asked to complete first a Referral form for triaging.

We prefer referrals from your GP or other health professionals.

On acceptance of referrals, please complete an Intake Form when your booking is confirmed.

We do not provide crisis or after hours services.

If you need urgent or crisis help, please contact your local Crisis Mental Health Services or the Emergency Services.
